Information for Patients
The NHS is in place to provide the best possible health and social care for all. It is to offer safe and effective care, when and where people need it; and empowering people in their choices. Every patient matters and that is why you may be asked to take part in a patient feedback questionnaire to help measure and work towards improving the quality of patient care. The survey comprises of 18 - 21 questions and a written comments section and should take no more than 5 minutes. Your cooperation is voluntary and completely anonymous.
The importance of your input – Your voice matters
NHS organisations continually want to improve the quality of patient experience. It is therefore paramount that they gain patient feedback to find out from first hand experiences where they are achieving well or failing in the NHS system.
The feedback gained is used to help develop and succeed in providing quality care for all within the NHS. Gaining insight into your thoughts can not only help get a greater picture of care provided but help in setting clear actions to eradicate potential issues that may arise from poor patient experience.
Your feedback and comments
The Patient Perception questionnaire consists of two forms. The first form comprises of multiple questions with tick box response system. The questions rate specific areas of personal development which are focussed around patient experience and care. The second form contains three comments boxes where written feedback can be left. This part gives you the opportunity to provide additional comments to support or add value to your feedback responses from the previous page. It must be kept in mind when completing the written feedback section that your comments will not be edited so you must avoid using personal details or offensive language.
When you have completed your form you are to place both pages in the Patient Perception Box which you will find on the ward/surgery reception or alternatively you can hand into a member of staff who will do this on your behalf.
Your two form survey is then sorted by a member of the trust. The comments form is kept on site for review and the multiple question form is sent to Medicology Ltd who, as an independent organisation, sort and scan the forms for reporting. The report of results is then made available to the participant and their organisation for review.
Publishing the results of a completed Patient Perception study is at the discretion of the organisation. Openly publicising these results is encouraged and thought to be of benefit to both the NHS and its users.
