I’ve lost my Patient Perception form
If you wish to complete the Patient Perception questionnaire but have mis-placed your form, please contact the hospital, surgery or other organisation and they will be able to provide you with another copy.
I can’t seem to log in
If you have entered the codes as they appear on the Patient Perception form and image verification number on the website but still can't log in, please contact explaining your problem and we'll be more than happy to help.
How long does it take to complete?
Completing the questionnaire either online through this website or by filling in the paper version should take no longer than 5 minutes - less for most people.
Is this anonymous?
Yes, completely. The codes used on the forms only identify the particulat hospital or organisation you visited, the department and the staff member who issued you the Patient Perception form. No information about you apart from your gender and year of birth are recorded. Your answers will always remain confidential and anonymous.
